What is a sports or school physical?
A sports or school physical, also known as a pre-participation physical examination (PPE), is a requirement for participation in certain athletic and school programs. The purpose of the physical is to determine whether or not a child (or adult) is physically able to participate in a sport or program safely.
Sports and school physicals do not replace and are not substitutes for routine, annual physicals performed by a Primary Care Physician (PCP) or Pediatrician.
What do I need to bring for a sports or school physical?
Your school or sports program will typically provide you with their designated Physical Form. Please bring this physical form in and fill out with as much detailed health history, certifications, consents, and any applicable records and/or medical history that would impact your or your child’s ability to safely participate in the school or sports program (i.e. heart problems or heart conditions).
